This is a “Bench Design & Signature Form” filled out by the American Cancer Society student organization. The document includes drawings and designs for bench painting, and it features sketches of images and symbols that reflect the organization’s mission and values. The first panel is the Back Rest/Front View. On the left is a drawing of two orange serpents entwined around the base of a blue sword, representing the logo of the American Cancer Society. On the right is “American Cancer Society” in blue. Both are set against a white background. The middle panel is the Seat View, and it depicts 10 ribbons representing ten forms of cancer and cancer awareness. The drawing is outlined in blue marker intended to represent a blue background. There is a black ribbon representing melanoma, a pink ribbon representing breast cancer, a yellow ribbon for bladder cancer, a brown ribbon for myeloma, a white ribbon for lung cancer, an orange ribbon for leukemia, a purple ribbon for pancreatic cancer, a blue ribbon for ovarian cancer, a green ribbon for kidney cancer, and a gray ribbon for brain cancer. On the bottom left is “@gmu_acs” and a drawing of the Instagram logo, labeled as colored “white.” The bottom panel is the Back Rest/Back View. It reads “Help Us End Cancer As We Know It For Everyone.” The words sit in the center of the panel; notes on the form indicate the words will be painted white and set against a red background that fills the rest of the panel. Below the bottom panel is written “3 main colors: white, blue, red.”
