Description

This is a “Bench Design & Signature Form” for the student organization “Financial Planning Association.” Instructions for applying organizations are listed under the form heading. The next portion of the application has three sections for organizations to illustrate a bench design: “BACK REST/FRONT VIEW,” “SEAT VIEW,” and “BACK REST/BACK VIEW.” The image depicts three hand drawn sketches of different views of the organization’s bench. Below the design portion there are three categories of requested organization information: “Organization President Signature,” “Date,” and “Print Name.” The organization president hand signed the application. The date is listed as “3/21/2023,” and the print name is “Nicholas Couidos.”

The last section lists “Drawing guidelines” for the organization applying.
