This is a two-page “Bench Design & Signature Form” for the student organization “Every Nation Campus.” Instructions for applying organizations are listed under the form heading. The next portion of the application has three sections for organizations to illustrate a bench design: “BACK REST/FRONT VIEW,” “SEAT VIEW,” and “BACK REST/BACK VIEW.” The image depicts three hand drawn sketches of different views of the organization’s bench. This bench design depicts the sky and ground to relate to the chosen Bible verse on the bench.
Back Rest/Front View: This view is divided into three equal horizonal sections all with a blue background. White clouds are drawn within each section, with “EVERY NATION CAMPUS GMU” written in white cloud letters from left to right in the top section. To the right of the letters in the right-most corner is a small yellow sun. The bottom section has “No power in the sky above or in the earth” is written in white cursive letters against the blue background.
Seat View: The background for this view is green and written in white cursive is “below – indeed, nothing in all creation will ever be able to separate us from the love of God that is revealed in Christ Jesus our Lord.” Below this text to the far right is “Romans 8:19” written in white cursive letters and Arabic numerals.
Back Rest/Back View: The background for this view is blue with “ENC” written in large white and blue letters in the center and “EVERY NATION CAMPUS” in white letters beneath.
Below the design portion there are three categories of requested organization information: “Organization President Signature,” “Date,” and “Print Name.” The organization president hand signed the application. The date is listed as “3/24/23,” and the print name is “Navy Powell.”
The last section lists “Drawing guidelines” for the organization applying.
