Description

This is a “Bench Design & Signature Form” for the student organization “Freedom Connection Coalition.” The background color of the application is tan. Instructions for applying organizations are listed under the form heading. In the far right of the bottom section is “@freedomconnection.gov” written in blue letters.
Back Rest/Back View: This view is divided into three equal horizontal sections. In the top section, from left to right is “NATIONAL HUMAN TRAFFICKING HOTLINE” written in green letters. The middle section has “(1) 888 373 7888” written in blue letters. In the bottom section are black hand drawn dots connected by black lines.

Below the design portion there are three categories of requested organization information: “Organization President Signature,” “Date,” and “Print Name.” The organization president hand signed the application. The date is listed as “3/24/23,” and the print name is hand signed “Elise Houser.”

The last section lists “Drawing guidelines” for the organization applying.
