Description

The document details a completed “Bench Design and Signature Form” application for the “Phi Delta Theta” organization. The document is a digital scan of a physical application. Instructions depicted at the top and bottom of the application inform applicants of the necessary requirements for a completed submission. The document depicts the symbol of a sword and shield, as well as Greek and English letters in multiple shades of blue and white.
The BACK REST/FRONT VIEW depicts a sequential background of shades of blue, ranging from dark blue to blue and then to a lighter blue. This sequence originates from the outside of this section and becomes a lighter shade of blue as it continues toward the center. These different shades of blue are separated by a black outline. Directly in the center of this design section are the Greek letters “Phi, Delta, Theta,” written largely in white.
The SEAT VIEW depicts a sequential background of shades of blue, ranging from dark blue to blue and then to a lighter blue. This sequence originates from the outside of this section and becomes a lighter shade of blue as it continues toward the center. These different shades of blue are separated by a black outline. In the center, the symbol of a sword across a shield is visible. The sword and shield are dark blue, outlined in white. On both sides of the sword are white stars, three on each side, totaling six.
The BACK REST/BACK VIEW portion of the submitted design features a blue background, with the word “Signatures” written in parentheses. The color gray is used for this writing. The “(Signatures)” indicates an area where the organization’s members would sign their names.
